Polk County’s Health and Human Services Division will host a Community Health Fair on Tuesday, Nov. 12, from 9 a.m. to noon at The Mission in Winter Haven. Meet with Polk HealthCare Plan representatives and receive the following health screenings and services at no cost to you:
- Chronic disease screening
- Healthcare navigators
- Health education
- Hearing screenings
- Behavioral health resources
